To order by mail, download and print the order form below. Enclose the form along with a check or money order payable to the Clinton County Health Department for the cost equal to the number of test kits you are ordering. There is no additional cost besides the test kit – these prices include the postage-paid test kit, shipping, analysis of the kit(s), and a report to the end user.
